You will book a “New Patient Evaluation”.

Are you a current patient?

You will book either the “Current Patient Follow Up Session” OR a speciality visit which includes: Labor and Delivery Prep & Fertility Treatment Session. These sessions are for patients who have been seen in the office within the last 12 months. If you have not been seen in a year, please schedule a “New Patient Evaluation”.

Policies

• Cancellation Policy: Cancellations with less than 48 hours notice will incur a cancellation fee of 50% of the cost of your visit. Cancellations with less than 12 hours notice will incur a cancellation fee of 100% of the cost of your visit.
• Insurance: We are an out of network practice, which means we work for you, not your insurance company. We do not contract with any insurance companies, which enables us to provide you with the care you need, not simply what your insurance will pay for. We are happy to provide a super-bill, which is an itemized receipt you can submit to your insurance company for reimbursement. Credit Card + FSA/HSA cards are accepted forms of payment.
